What is an AFFIDAVIT?

It is a form in which a student attending a school within the district, resides with his or her parent(s), legal guardian(s), and/or care taker(s) , but whose residence is located in the home of another adult living within the boundaries of the Georgetown County School District. This form is to be submitted annually before the start of each school year or upon moving into the attendance area of the new school.

All completed forms must be submitted along with all required documents such as a current utility bill ( gas, electric, and/or water) in the name of the home owner.
