Immunization Record

 

Most of the time we do NOT have Immunization Records.  Immunization records are the property of parents/guardians and were not considered a part of a school record until the Georgetown County School Board mandated it to be kept in a student's record after 1990.  Even after the mandate, if a parent/guardian did not provide the school with a copy of the student's Immunization Record, it will not be in the student's archived cumulative record.

In order for our office to conduct a search for your Immunization Record, you will need to complete a VITAL RECORDS REQUEST (link provided below) .

If your Immunization Record is available, it will be sent to you.  If the record is not found, you will be issued a refund.

If you are currently attending a school in Georgetown County or attended school in Georgetown County less than three (3) years ago, you will need to contact the school you LAST ATTENDED for a copy of your Immunization Record.

NOTE:  THE RELEASE OF AN IMMUNIZATION RECORD REQUIRES A SIGNED REQUEST FROM THE STUDENT.  PARENTS/GUARDIANS MAY NOT SIGN FOR THE IMMUNIZATION RECORD IF THE STUDENT IS OVER THE AGE OF 18.  PARENTS/GUARDIANS MAY REQUEST A COPY OF THE IMMUNIZATION RECORD OF THEIR CHILD IF THE STUDENT IS LESS THAN 18 YEARS OF AGE.  TELEPHONE CALLS WILL NOT RELEASE THE IMMUNIZATION RECORD.
